Tijdsconflicten in het ecosysteem tussen zorgverleners, organisatie en patiënten met chronische aandoeningen

Healthcare providers are a key component in the care of chronically ill patients. These patients live with long-term conditions and therefore maintain ongoing contact with healthcare providers, who offer support, answer questions, and provide guidance. However, healthcare providers are often under time pressure due to increasing time-related demands, such as patient needs and administrative tasks. As a result, they may be unable to spend the necessary time on, for instance, in-depth conversations about mental health aspects. This is frustrating not only for chronically ill patients, but also for the healthcare providers, who want to offer the optimal care their patients need.
In this thesis, the time conflicts healthcare providers face in their daily work are identified. The internal experience of time from the provider’s perspective is examined in more detail, referred to as intrasubjective time. Additionally, differences in time perception between the healthcare provider and their immediate or broader environment are explored, which is referred to as the provider’s ecosystem. In this study, the ecosystem includes time conflicts between the healthcare provider and other providers, chronically ill patients, and the healthcare organization (hospitals). When there are differing perceptions of time, such as in scheduling appointments or the duration of consultations, intersubjective time conflicts may arise. Patients may feel they did not receive all the answers they needed, while healthcare providers may feel the consultation was too short, but still have to end it because another patient is waiting.
The time conflicts were identified through interviews with healthcare providers who care for patients with chronic conditions, specifically cancer, Crohn’s disease, or kidney failure requiring dialysis. During the interviews, providers were asked about their experiences and insights regarding time conflicts with their ecosystem. The conversations also explored how these conflicts made them feel and how they managed them in daily practice. The interviews revealed a range of time-related challenges, from communication issues and administrative burdens to mismatched expectations. The findings suggest that improvements are needed to better support healthcare provider’s workflows. Without these changes, providers risk becoming overworked, which can lead to a decline in care quality. Potential improvements include better communication with the organization, and stricter time slots with breaks in between so providers have time for other tasks or can extend consultations when needed. Additionally, it's important for management to occasionally visit departments to observe firsthand and address the time pressures highlighted in the interviews, considering both the management and healthcare provider perspectives. Addressing these time conflicts will help improve overall healthcare quality.
